To be honest, I believe it is different in every country you are in. I can only speak about my experience with the Swedish system, but I hope it can give you some insight.
Here in Sweden you are supposed to go through a year long investigation where you meet a counselor, a psychologist and a doctor who will all make their separate judgements about you and your state of being. They will ask about how you view yourself, about your childhood and how it has been growing up. How you feel right now and how you feel about the future, all with regards to your assigned and your identified gender.
The investigation can take longer, or it can take shorter depending on how long it takes for the different doctors to get a clear image and make a judgement call of whether they believe transitioning will be the right way for you to continue from there.
I started this investigation in September last year, and got my first shot of testosterone in July this year. Before starting the investigation I had to go to a regular psychologist and make them send my case on to the investigation that specializes in transsexualism. It took maybe 2-3 months, and then I didn’t have my first meeting with the investigators until another 3 months later.
When it comes to payment I had to pay for my appointments until I reached the payment roof for healthcare. It’s part of the healthcare here in Sweden. You pay a certain sum yourself and when you’ve reached the roof on it you get a card that allows you free appointments at doctors for a year.
I also have to pay for my testosterone shots, but the roof principle applies here too. I only have to pay a certain amount before I get it free, and the closer I get to that maximum sum the more percentages of reduced price to I get on the medicine.
Since the T-shots are quite expensive, I am up in the roof amount after only 2 shots, but they are luckily on monthly pay-off. I pay a bit each month instead of the whole sum at once.

since i’ve now seen this concern raised twice in the past five minutes:
alternate sources of calcium for lactose intolerant estrogen-takers include nuts (almonds and hazelnuts are probably easiest to get), beans, broccoli, and kale
alternate sources of vitamin c for testosterone-takers allergic to oranges include what seems to be nearly every other fruit or vegetable (of which i’m going to arbitrarily highlight chili peppers, parsley, and broccoli again), and liver if that’s something you’re willing to eat
Decided to blacklist a lot of da3 tags today because I don’t want to kill my current state of attitude towards the game and idk, I simply don’t want to see more da3 stuff on my dash before I play the game myself. (and I don’t really have to explain why I chose to do it, but there you go)

If I can get my depression a little bit more under control (I’ve had some lighter version of symptoms I haven’t had in 2 years, back during my worst period) and if I can keep my life in order what with unemployment requirements and financial aid and if I can also finally get a date for my surgery, I will try and do an art giveaway again.
Should be up in time for inquisition I suppose, if things go well, that is.

I don’t know about later, but not at this time. I’m kind of struggling with my depression again so I don’t want to draw something unless it’s something I do for myself. (also this screen is really dark so the colors are all messed up)
